Guidewire introduction

Guidelines:

  • The ARROW and Terumo cannula is compatible with 0.025" wires, the KIMMAL-needle with 0.035" wires.

  • Do not use straight wires; shape a "J".

  • Never force the wire against resistance since the wire may take a false route.

  • If resistance is met, check by fluoroscopy or angiography.

 

  • If the 0.025" wire cannot be advanced beyond the brachial artery, introduce sheath first, followed by exchange for a 0.035" Terumo (hydrophilic) wire.

  • When hydrophillic wires are used, be sure not entering small sidebranches.

  • If the wire enters the descending instead of the ascending aorta , retry at deep inspiration or direct wire by the guiding catheter.
